Emergency Contact Coordination ensures that in a crisis, you are not scrambling for phone numbers or trying to remember who to notify first. We work with families and care teams to identify key medical providers, pharmacies, neighbors, relatives, paid caregivers, and facility contacts.

We then organize this information into an easy-to-follow communication plan that clarifies who calls whom, in what order, and for what type of situation (fall, hospitalization, evacuation, missing person, power outage). Copies can be kept at home, with caregivers, and in a go-bag or electronic form. For individuals living with dementia, we also recommend ways to discreetly carry identification and emergency information in case they become lost. This service turns scattered information into a reliable safety net so responders and loved ones can coordinate quickly and effectively.
